Purpose of Job:

The primary purpose of your job position is to provide pastoral care and emotional support to White Horse Village residents with a primary focus on the needs of residents in the Health Care Center. The Pastoral Care Coordinator develops and leads, when appropriate, religious activities and groups for the residents and staff in the Health Care Center. The Chaplain also assists with connecting residents with spiritual resources in the greater community as needed and working with both the interdisciplinary care team as well as outside agencies such as hospice where indicated.

Administrative Functions:

Maintain a good work rapport with inter-departmental personnel.

Assist in planning, developing, organizing and implementing departmental activities as directed.

Assist coordinating departmental service with other departments.

Adhere to all WHV policies and procedures.

Perform administrative duties such as: filing, completing forms, reading memos and attend meetings, as directed.

Create and maintain a good customer service environment.

Training/In-Service Attendance:

Attend and participate in all required in-services and educational classes.

Attend and participate in workshops, seminars and coursework, as directed.

Safety and Sanitation:

Follow and establish safety procedures and precautions when performing the aspects of your job.

Report all injuries immediately to your supervisor.

Lift using proper technique. Assist others when directed.

Report all unsafe and hazardous conditions and equipment to your supervisor.

Maintain a work area that is clean and hazard free.

Follow all infection control procedures as they apply.

Know and follow fire and disaster plans.

Use personal protective equipment when performing tasks that might involve infectious materials.

Equipment and Supply Usage:

Report all equipment malfunctions or breakdowns immediately.

Report low or depleted supplies to the appropriate person.

Use equipment for its intended purpose.

Follow correct official procedures when using equipment.

Resident Rights:

Maintain the confidentiality of resident information.

Handle all resident property with respect.

Knock before entering into a resident’s room or dwelling.

Ensure that residents are treated fairly and with respect allowing for individuality and privacy.

Uphold the residents’ civil rights.

Regulatory Compliance:

Follow all OSHA, CDC guidelines.

Review book of Material Safety Data Sheets prior to working with any chemicals.

Treat all team members fairly and equally without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status.

Adhere to all local, state or federal regulations as they apply.

Completes annual compliance training and exhibits behavior as set forth by the code of conduct in the performance of their duties.

Abides by White Horse Village’s code of conduct in the performance of their duties.

Committee Function:

Serve on Assessment Committee, Spiritual Life Committee and Village Neighbors.

When serving on a committee, participate to the fullest of your ability.

Health Care Administrative Functions:

Responsible for planning, scheduling and evaluating all spiritual care activities within the health care center in accordance with the Mission, Vision, Values and Health Care Philosophy of White Horse Village.

Recommend and develop policies and procedures related to spiritual care at White Horse Village to ensure spiritual needs of health care residents are met.

Build relationships of trust through visits, presence, counsel and conversations with residents, team members and others.

Chart spiritual intervention and share concerns with care team.

Participate in appropriate interdisciplinary team meetings, as requested by resident/family.

Provide spiritual assessment toward coordinated care of the resident (on a referral basis.).

Assist in counseling of residents in adjusting to new status (transfers from IL to PC or Skilled).

Provide consultation related to ad hoc health care ethics committee.

Cultivate resource base of community clergy support.

Provide emergency religious resource list when particular faith group/denominational resource is requested. Update list as needed.

Provide education per the elderly and spiritual care issues; provide in-servicing to staff as needed and requested.

Provide support to residential community with issues related to loss, grief, caregiver support and other emotional needs as consulted.

Training and supervision of volunteers from religious communities who provide spiritual support to our residents.

All other duties, as assigned.

Resident Service Functions:

Provide spiritual and emotional support to health care residents and families

Lead or provide special services that are inclusive and welcoming to all faiths and denominations.

Provide empathetic listening.

Provide visitation to residents on an individual basis as requested by the residents or by referral of staff, family.

Coordinate end of life care support as needed; collaborate with hospice to provide end of life support as appropriate.

Conduct a monthly grief support group for healthcare residents.

Communicate residents’ wishes regarding end of life spiritual support to health care center staff.

Take appropriate initiative in developing pastoral relationships with residents.

Function as an advocate and/or mediator for residents, families.

Provide supportive services for resident transition or end of life care.

Provide or arrange for appropriate religious observances in health care center.

Manage and coordinate Memorial services with family members that are to be held in the Auditorium

Provide spiritual and religious resources for residents, families and staff.

Facilitate groups in the health care center; offer spiritual programs based on resident need and preference.

Performs all other duties as assigned.

Compliance Requirements:

  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of duties.
  • Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
  • Follows all local, state and federal regulations as they pertain to the position.
  • Adheres to the Organization’s Policy and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

Job Type: Full-time

Pay: $28.00 - $30.00 per hour
