BSW or related degree & long term care experience
Full time /40 hour work week.
Purpose of Position: The primary purpose of your job is assistance in the overall operation of social services in the Health Care Center, in accordance with current federal, state and local standards, guidelines and regulations and the established policies of White Horse Village. The Health Care Social Worker (HCSW) works collaboratively with the health care social service coordinator and all departments to advocate for residents in the Health Care Center. The HCSW provides emotional support to both residents and families. The HCSW addresses the medically-related needs of residents and assists residents in meeting their highest level of psychosocial functioning. The HCSW assists in coordinating discharge planning as appropriate. In addition, the HCSW will assist the Health Care Admissions Coordinator in in admissions processes for the health care center.

Administrative Functions:
- Maintain a good work rapport with inter-departmental personnel.
- Assist in planning, developing, organizing and implementing departmental activities as directed.

- Training/In-Service Attendance:

- Safety and Sanitation:
- Follow and establish safety procedures and precautions when performing the aspects of your job.
- Report all injuries immediately to your supervisor.
- Lift using proper technique. Assist others when directed.
- Report all unsafe and hazardous conditions and equipment to your supervisor.
- Maintain a work area that is clean and hazard free.
- Follow all infection control procedures as they apply.
- Know and follow fire and disaster plans.
- Use personal protective equipment when performing tasks that might involve infectious materials.
- Equipment and Supply Usage:
- Report all equipment malfunctions or breakdowns immediately.

- Follow all OSHA, CDC guidelines.
- Review book of Material Safety Data Sheets prior to working with any chemicals.

Leadership Functions:
- Work collaboratively with the health care social services coordinator to plan, develop, organize, implement, evaluate and direct the functions and operations of the social services program for the Health Care Center.
- Make written/oral reports to the Director of Social Services concerning the Health Care Center social services program operation/policies/procedures.
- Assist in developing methods for coordinating social services functions with other departments.
- Other duties as deemed necessary and appropriate by the Director of Social Services.
Resident Services Functions:
- Serve as a resident advocate. Review concerns received / made by residents/families and provide a written report to appropriate departments immediately. Review all concern resolutions with Administration and maintain in a master binder.
- Inform residents/families of personal and property rights during their stay in the Health Care Center.
- Provide assistance with new resident/family orientation and offer emotional support to residents/family members during the adjustment process.
- Assist in coordinate resident discharges from the Health Care Center. Make referrals to home care agencies, the Resident Support Service Program, medical equipment companies, etc… as indicated and review plan with resident.
- Assist in coordinate discharge plan with other departments such as nursing, outpatient, transportation and notify family of plan.
- Serve as a back-up to the Health Care Center’s Social Services Coordinator.
- Serve as a back-up to the Health Care Admissions Coordinator.
- Participates in the development of resident care plans and attend interdisciplinary team meetings.
- Assist with advance directive education as needed.
- Assist in coordination of mental health services and other ancillary support services as indicated.
- Provide staff in-servicing as needed/requested.
Documentation Function:
- Complete documentation requirements in accordance with established policies and current skilled nursing / personal care facility regulations.
- Interview / assess all residents and complete social history and initial admission assessments as required upon admission.
- Complete all MDS documentation as required.
- Track and document all direct care as provided (by the position) and as required for PBJ (Payroll-Based Journal).
- Complete all documentation in accordance with the current standards of practice for social workers.
- Complete admissions documentation in accordance with community practices and regulatory requirements.
- Quality Improvement function:
- Assist in review of social services program, policies, procedures, manuals, job descriptions, etc… at least annually and implement recommended changes.
- Assist in tracking resident and family concerns, monitoring for trends and implementing plans for correction.
- Assist with audits of social service documentation to ensure compliance with regulatory requirements. This may include and is not limited to Medicare cut letters, resident’s rights and advanced directives.
- Assist with audits of admissions documentation, files, etc. to ensure compliance.
- Monitor resident / family satisfaction both informally and formally through surveys.
- Assist with annual DOH survey preparation.
- Participation in inter-disciplinary QAPI projects.
