An overview of this position entails…This position is responsible for medication management for the residents, coordinating health and wellness initiatives, and maintaining communication between health care providers and the residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Demonstrate ethical and professional behavior that align with the values of The Commons and The Fountains of Hilltop, including Service 1, how we treat each other, and Service 2, how we treat our residents. Service 1 and Service 2 is demonstrated by setting boundaries, being predictable, consistent, kind, and transparent.
2. Conducts oneself as a leader by taking ownership of the program and providing quality of care; commits to the betterment and growth of ALCs by offering solutions, developing processes, promoting communication with others, supporting communication from staff to management, prioritizing tasks, and actively participating in problem resolution.
3. Provide timely feedback and training to leadership, management and front-line staff regarding performance including weekly activity reports, software reports and other reports as requested.
4. Provide direct supervision for assigned departments. Duties include hiring, scheduling, evaluating, counseling, terminating, and ensuring effective work performance, providing appropriate performance feedback, addressing performance concerns, and conducting performance evaluations in a timely and effective manner.
5. Regular and on-going audits and maintenance of resident medication records and medication profiles including mediations kept at bedside. 6. Ensuring compliance with doctor’s/pharmacy orders via phone, fax, and other communication tools.
7. Tracking and review of medication errors, follow up with staff, and correct reporting as needed.
8. Conducting illness and injury assessments or outbreaks on residents and staff as necessary; documenting and reporting as necessary. Provide emergency first aid, as required.
9. Assuring facility compliance with licensure requirements.
10. Maintaining effective communication between all parties involved with residents (i.e., doctors, pharmacies, family members, hospice, home care, etc).
11. Ensuring resident medications are legal, safe, and accurate. Identifying negative drug interactions, and/ use within prescribed guidelines.
12. Educating staff on medication uses, types of administration, and quality assurance topics.
13. Delegation of care to care specialists as needed and within regulations.
14. Creating, reviewing, and revising policies as needed.
15. Reviewing/creating Medication Administration Records for newly admitted residents.
16. Writing assessments and/or referrals for hospice and home care and maintain communication of services being provided.
17. Provide initial and on-going training to the Medication Assistant(s). Maintain appropriate database and records for residents utilizing established technology.
18. Collaborate and assist with other programs in the organization to provide support to residents and staff, as needed. Check Tuberculosis skin test results, as needed.

EDUCATION & EXPERIENCE
Current Licensure in good standing as a Registered Nurse (RN) in Colorado, and two (2) years of related work experience required.

• Valid Colorado Driver’s License (required)
• Proof of current automobile insurance (required)
• CPR Certificate/First Aid Certificate (required)
• De-Escalation Training (preferred)
• Hilltop’s Transportation Certification (preferred)
