An overview of this position entails…Responsible for advocating for and coordinating person-centered care for residents and participants of the Life Adjustment Program (LAP) through communicating with residents on a regular basis, maintaining contact with natural supports, working closely with other teams and disciplines in the care plan, and creating opportunities for residents in the surrounding community.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Demonstrate ethical and professional behavior consistent with Hilltop's Life Adjustment Program values, including Service 1, how we treat each other, and Service 2, how we treat residents. This involves setting boundaries, being predictable, consistent, kind, and transparent.
2. Conduct oneself as a leader by taking ownership of the program, ensuring quality of care and commitment to the betterment and growth of the Life Adjustment Program. Offers solutions, develops processes, promotes communication with others, and supports communication from staff to management. Prioritizes tasks and brings concerns to supervisors/managers when necessary.
3. Provide person-centered training and assistance in accordance with designated care plan (i.e., selfcare, benefit coordination, medication reminders, etc.), within scope of practice.
4. Understand and strictly follow all applicable Hilltop Corporate, State and Federal rules, regulations, policies and procedures.
5. Complete required documentation promptly, accurately, and legibly.
6. Provide behavior management as required (i.e., defusing anger). Independently create and implement interventions for residents who many have increased difficulty integrating into the milieu.
7. Facilitate and participate as member of the routine Multi-Disciplinary Team (MDT) process and ensure updated communication and compliance with process. Facilitate and coordinate interim MDTs, as needed.
8. Create and maintain positive working relationships with residents, peers, leadership, and all stakeholders.
9. Communicate regularly with residents using frequencies and methods that ensure ongoing communication regarding needs, wants, and wishes.
10. Develop and implement a system that ensures case manager is able to manage resident’s data and property to include moves, purchasing of clothing, and other necessary items.
11. Act professionally as the resident representative for the program on all identified issues/purposes.
12. Participate in the administration of assessment and interventions to ensure appropriate level of support and protective oversight is provided to each resident.

EDUCATION & EXPERIENCE
Bachelor’s degree in a human services-related field, or equivalent related experience working in the human services field; qualifying experience may include case management, working with adults in a residential setting, or working with people having experienced traumatic brain injury.

• Valid Colorado Driver’s License (required)
• Proof of current automobile insurance (required)
• CPR/First Aid Certificate (preferred)
• De-Escalation Training (preferred)
• Hilltop’s Transportation Certification (preferred)
