JOB SUMMARY :
The Social Worker shall meet the psychosocial needs of the individual resident and / or their families by providing services which will assure the highest practical quality of life for the residents by promoting the residents’ emotional and physical well-being, encouraging self determination and self respect and by insuring dignity. The Social Worker shall participate in interdisciplinary care plan meetings, interpret and uphold Residents’ Rights and advocate for the residents with personal and social problems.
EDUCATIONAL BACKGROUND :
Degree in Social Work or related area.
KNOWLEDGE, SKILLS AND PERSONAL QUALIFICATIONS :
1. At least 5 years’ experience in a nursing home facility with knowledge of gerontological problems and familiarity with community agencies and resources.
2. The ability to relate to long-term care individual’s needs.
3. The ability to carry out assigned tasks to completion.
4. Good communication skills; must understand English; written, oral and listening.
5. Must be able to observe and demonstrate proper safety and sanitary techniques.
6. Must be a team player, able to work independently, and with a wide variety of individuals.
7. Maintains a responsible and positive attitude.
8. Good organizational skills.
9. Must be able to multitask.
RESPONSIBILITIES OF DAILY FUNCTIONING :
1. Assist in meeting the psychosocial needs of each resident, their designated representative and / or family as appropriate or necessary in coping with the effect of illness, disability, treatment, or stay in the facility
2. Participation in resident care management.
3.Experience with discharge planning and MDS.
