Nurse Care Coordinator
Position Overview
The Nurse Care Coordinator will be responsible for overseeing the transition during hospital discharge. The NCC will be responsible for making sure all patients receive a timely TCC follow up appointment with their Primary Care Provider as well as assist our Hospital Partner with ensuring that patients without a Primary Care provider receives follow up care as well. Assist in patient placement for SNF’s, Home Health and other services recommended by the discharging Provider. Responsible for building and maintaining a partnership with our Hospital Partners.
Primary Responsibilities:
Patient Discharge Coordination
- Collaborate extensively with hospital case managers, nurses, and social workers to:
- Identify appropriate post-discharge care needs (TCM, SNF, home health, etc.)
- Ensure timely and appropriate patient transitions to Primary Care and other referred services by making sure the patient has scheduled PCP visits within an appropriate timeframe post-discharge
Communication and Documentation:
- Maintain daily communication with White-Wilson Transition of Care team members to ensure the patient transition is smooth and complete prior to the visit.
- Ensure complete and accurate transfer of:
- Discharge paperwork
- Comprehensive medical records
- Relevant patient care documentation
- Verify all necessary medical information is properly compiled and transmitted
Interdisciplinary Collaboration with our Hospital Partner:
- Collaboration and Communication will be key in ensuring our patients are discharged with the correct services and resources to prevent readmission. It is vital that we work with all applicable parties to ensure our Primary Care Provider has all necessary records for a smooth transition back into the practice.
Qualifications and Requirements
Educational Background
- Current, active Registered Nursing (RN) License
- Bachelor of Science in Nursing (BSN) preferred
- Case Management Certification desired
Professional Experience
- Minimum 2-3 years hospital or acute care nursing experience
- Proven track record in patient care coordination
- Strong understanding of healthcare discharge processes
