NP of Physician Assistant

Nurse Practitioner of Physician Assistant needed for outpatient FQHC community health system to provide primary care services at Queens center with 1 day a week floating to satellite locations. Job Function

• Providers provide routine primary care, wellness assessments and refer for specialty care
• Join a location with other providers and a pharmacy onsite
• Provide patient care to patients of all ages (pediatric, adolescent, women's health and adults).
• Monday-Friday schedule, 9:30am-5:30 pm with NO WEEKENDS
• Evaluate patients medication needs, write prescriptions
• Communicate with case workers, RN teams and other ancillary staff to ensure treatment plans are carried out
• Provide patient education around health promotion, disease prevention, and health maintenance
  
  
  

Job Requirements

• NY State license, DEA, CSR (active or applied preferred)
• Training as a Nurse Practitioner or Physician Assistant
• 1 year of primary care experience
