Base pay of $19.50/hour with $1/hr shift diffierential for 2 p to 10 p; $1.50/hr overnights. $21/hr if CNA, points given for experience
Position Summary:
Supervised by the Director of Clinical Services and responsible for carrying out nurse delegated tasks, the Resident Assistant.
- Completes required training. Communicates with the nurse and accepts the responsibility of being accountable for performing delegated nursing tasks.
- Assists in the provision of person centered care for Assisted Living and Memory Care resident at Wildwood Grove
- Get to know residents habits and routines to individualize care for each resident
- Encourages independence but is there to cue residents as needed
- Patience, kindness and a commitment to providing excellent care
- Must be a team player, helping co-workers, being on time, being dependable
Position Qualifications:
Preferred but not required:
- Successful completion of a Nursing Assistant/Home health Aide 75-hour course from state certified program or certified nursing assistant or Home Health Competency test scores of 80% or higher.
- Must be 16 years old
- Experience in an assisted living, skilled nursing or memory care preferred but not required. We will train the right person who is willing to make a commitment.
- Demonstrated ability to read, write, and carry out directions.
- Evidence of maturity and ability to deal effectively with job demands.
- Good verbal and written communication skills required.
Position Responsibilities:
1) Appropriately assists clients with personal care activities in accordance with the personal care plan including: a. Baths (tub, shower, bed, bath) b. Mouth care, denture care, skin care, foot care c. Grooming (shampoo, dry and comb hair, shave, filing only of nails) d. Elimination needs (bedpan, commode, toilet, urinary drainage bags) e. Transferring (bed to chair, to wheelchair, to tub, to shower) f. Dressing and undressing activities g. Ambulating
2) Appropriately assists with household tasks directly essential to the client’s health and in accordance with the personal care plan including: a. Planning and preparing nutritious meals b. Assisting in feeding client as directed c. Light housekeeping of client’s immediate environment d. Change linen, make bed e. Laundry of client’s clothing and bedding
3) Accurately measures and records oral, tympanic, rectal, axillary temperatures, pulse, respirations and blood pressure when ordered (within the training limitations of the aide).
4) Demonstrates appropriate and safe knowledge/use of equipment (stands, Hoyer lifts, side rails, wheelchairs, oxygen, etc.) within training limitation.
5) Performs range of motion and other simple procedures as an extension of therapy service as ordered (within training limitation).
6) Assists the client in medication assistance/administration as delegated by the Registered Nurse. i. Provides medication assistance/administration from original packaging or medication box preset by nurse. ii. Observes and reports medication assistance/administration on appropriate documentation.
1. Follows the Medication Administration Record (MAR) in administering medication to clients.
2. Checks at beginning of each visit for client medication updates by reviewing Medication Administration Record
(MAR).
3. Prepares medication for each client’s medication administration with attention to the following: a. Right resident b. Right drug c. Right dosage d. Right time e. Right route f. Check expiration date

4. Assures that client takes medications as ordered by prescriber.
5. Ensures confidentiality of client medication among clients and family members.
6. Keeps medications secured in designated storage area.
7. Documents legibly all client concerns made while administering the medication.
8. Completes client cares as outlined by the nurse and delegated to the home health aide.
9. Verifies inventory of controlled medications as directed.
10. Maintains orderliness and cleanliness of medication designated storage area.
11. Notifies nurse if medications or supplies are needed.
7) Performs other service appropriate for the discipline, as requested by the Registered Nurse or designee, and/or directed via the personal care plan.
8) Accurately and thoroughly documents service provided in accordance with agency policy and the personal care plan on the day services are rendered
Note: Submission of clinical, time and any other assignment related documents is required for satisfactory completion of an assignment as defined by Wildwood Grove policy. Failure to provide required documentation on a timely basis as defined by policy will result in disciplinary action, and in the case of time documents. Wildwood Grove will be unable to generate a paycheck due to lack of written documentation of time worked.
9) Immediately reports any changes in client’s condition or incidents to the Registered Nurse.
10) Establishes and maintains effective communication and a professional relationship with client/families.
11) Participates in case conferences with other members of the health care team as appropriate.
12) Maintains confidentiality in relation to all clients, health staff and documentation. Seeks direction form the Registered Nurse when/if asked to share any client information with outside parties.
13) Participates in quality assessment and improvement activities as assigned. Communicates information that enables collection and root cause analysis of data to identify opportunities for improvement.
14) Maintains and enhances skill through attending appropriate staff development programs and in-services.
15) Meets or exceeds annual education requirements in accordance with Wildwood Grove policy, state/federal regulations.
16) Appearance is professional and complies with agency dress code.
17) Assignments are completed accurately and on time.
18) Maintains a cooperative manner towards client/family and all members of the health care team.
19) Listens and response to client’s needs in a sensitive and timely manner either through own initiative as appropriate, or by timely referral to the Registered Nurse.
20) Uses proper body mechanics when performing all cares and adheres to the client plan of care.
21) Reports any work-related injuries immediately (not greater than 24 hours) to the supervisor.

Physical Requirements:

2) Climbing, balancing, stooping, kneeling, twisting, simulations and repetitive movements and related activities are integral to the performance of this position.
