Requirements
- Associates Degree from a regionally accredited college or university in Education, Human Service, or related field.
- 2 years of related experience, or the equivalent combination of education and experience.
- Bachelor's degree in Education, Human Service, or related field.
- Experience working with people with disabilities is preferred.

Essential Duties and Responsibilities may include the following, depending on the departmental assignment and services provided. Other duties may be assigned.
- Carries out the supported work plan for Program participants by providing community based employment supports and assessments to individuals with disabilities one-on-one, or in small group settings.
- Collects, organizes, and analyzes information about the people supported through records, tests, interviews, and professional sources, to appraise their interests, aptitudes, abilities, and personality characteristics, for the purpose of employment planning
- Maintains regular, effective communication with individuals, their families, agency staff, non-agency staff, business community contacts, and colleagues to support the objectives of their plans.
- Consistently monitors the work performance and progress of people receiving services through regularly scheduled job site visits, face to face discussions and phone contact with job-seekers, employees and their employers.
- Attends and participates in planning meetings for people receiving services to discuss progress and provide collaborative support
- Provides instruction and assistance as needed to individuals in areas including but not limited to workplace expectations, career development, personal hygiene, use of technology, social interaction, organization, stress-management, mobility training, benefits management, and personal finances
- Models appropriate/acceptable social and work skills.
Additional Requirements:
- A valid New York State Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required.
- Daily access to an automobile is required. The position requires travel.
Salary : $18 - $23
