Description
Wiley Christian Behavioral Services is seeking a Full Time Case Manager.
No Weekends! Daytime Hours!
Wiley Christian Adult Day Services is proud to be providing an excellent mental health and substance abuse partial care day program for adults. The Case Manager is a key member of the team that is providing these services. The Case Manager ensures we are providing the highest quality of services to our consumers.
Essential responsibilities include but are not limited to:
- Organizes, plans and conducts treatment groups as indicated by client needs and with direction from Supervisor and Program Director
- Performs client assessments, and designs and implements individualized treatment plans
- Documents treatment plans
- Maintains a daily record of client group attendance and completes weekly progress notes on all assigned clients
- Maintains a current record of community services, resources, programs and materials which are accessible to staff and clients.
- Refers all critical knowledge of client status to supervisors and team members in a timely manner.
- Is knowledgeable of regulations governing the program.
Qualifications
- Possess a bachelor’s degree in human services
- Leadership skills and ability to make independent decisions
- Good written and oral community skills
- Able to facilitate therapeutic groups
