JOB SUMMARY
The Resident Services Coordinator refers, connects, and coordinates appropriate services both on and off campus for Independent Living residents, collaborating with medical care providers and community agencies to assist residents in navigating services while promoting safety, health education, programs, and monitoring compliance.
PAY
Starting pay rate for this position is $51,700/yr and is commensurate with experience.
ESSENTIAL JOB FUNCTIONS
- Reviews pre-admission screenings with prospective Independent Living clients
- Completes baseline assessment on all Independent Living residents and completes re-assessments on annually or as needed
- Assesses psychosocial needs of residents and develops and promotes plans to connect them with proper resources
- Assists in navigating the medical care system and coordinates multiple community and state agency services for the medical benefit of the client
- Refers and links clients to appropriate services and community resources within the system of care to promote safety, positive health outcomes, treatment adherence and greater self-sufficiency; monitors client’s follow-through
- Develops and maintains working relationships with community agencies; works in conjunction with medical care providers
- Participates and provides input in Resident Review; serves on inter and intra-departmental teams; attends meetings for the Resident Health & Wellness Committee; acts as a facilitator of the Transition Care Team
- Assists in coordinating discussions with families and residents on the need for changing levels of care when safety is a concern
- Promotes awareness of health education programs and services; assists in developing new programs in response to identified needs
- Assists with Advanced Care Planning
- Maintains case records for each resident served, including all assessments
- Evaluates effectiveness of care coordination service based upon resident and staff feedback
- Conducts continuing education activities as requested
- Contributes to printed and social media of Williamsburg Landing as requested
- Maintains flexible work schedule to meet the resident and family’s needs that may include working after traditional hours and on weekends
- Maintains a high level of ethical conduct regarding confidentiality and professional stature
- Performs other duties as assigned
QUALIFICATIONS
A degree in social work or nursing and training in recognizing and assessing the emotional and social needs of the senior population is required; a minimum of two years’ experience in the field of social work or nursing is preferred; valid Commonwealth of Virginia driver’s license and proof of motor vehicle insurance required
KNOWLEDGE, SKILLS, AND ABILITIES
Ability to read, write, and speak English clearly and articulately with excellent verbal and written communication skills; ability to meet deadlines and work with limited supervision; ability to work effectively independently and as a member of a team; ability to exercise sound professional judgement and to respond appropriately in an emergency; knowledge of and the ability to implement human relations skills including problem solving, counseling and assisting; proficiency in Microsoft Word and Excel and ability to conduct internet research
