Williston Care Center is seeking a dedicated and skilled Per Diem Occupational Therapist to join our In-House Subacute Rehab team. As a member of our multidisciplinary rehabilitation team, you will provide high-quality occupational therapy services to patients recovering from illness, injury, or surgery. The ideal candidate will have a passion for patient care and a strong commitment to improving the functional independence of our patients.
Key Responsibilities :
- Conduct evaluations to assess patients' physical, cognitive, and emotional functioning.
- Develop and implement individualized treatment plans focused on improving patients' daily living skills, mobility, and overall independence.
- Provide therapeutic interventions including activities to improve motor skills, strength, and coordination.
- Collaborate with physicians, physical therapists, speech therapists, and other healthcare professionals to create comprehensive care plans.
- Monitor patient progress and adjust therapy plans as necessary to meet patient goals.
- Educate patients and their families on techniques and strategies to improve function and independence.
- Document therapy services provided, including progress notes, treatment plans, and discharge summaries.
