Location : Lakeport, CA,
- Job Role : Assisting emergency room and inpatient identified patients with substance and alcohol treatment.
- Responsibilities include identifying resources, working with community partners, connecting patients with outpatient mental health services, and intervening in suspected abuse / neglect cases.
- Additional responsibilities include identifying and addressing social determinants of health, providing cross coverage for inpatient and birthing center, and participating in committee review hearings.
- Requirements : MSW degree (LCSW not required). Minimum of 1 year of ED experience. SUN and Case Management experience preferred.
- Working Hours : 5 x 8 M-F, 0900-1700 with flexibility to start at 0830, 0900 or 0930 for an 8 hr day.
- Additional conditions : No weekend or on-call requirements.
