Admissions Assessment Nurse
1.0 FTE, 80 Hours per Pay Period
Shift: 8 am- 4:30 pm
Weekends: Every 3rd
Holidays: Every Other
Position Overview:
The Admissions Assessment Nurse supports long-term care residents by facilitating smooth admissions and coordinating the care delivery processes. Identifies risks and needs of individual residents and incorporates appropriate approaches and documentation to ensure high-quality care. Admissions Assessment Nurse works closely with teams on each unit of Lake Winona Manor as a mentor and facilitator for the delivery and execution of the care plan process.
Essential Duties & Responsibilities:
- Performs thorough and timely admission, quarterly, and other MDS-related assessments as required by the State of Minnesota.
- Evaluate the resident holistically to identify risks, strengths, and needs while residing in the facility.
- Creates and implements resident care plans for new admissions.
- Ensures care plans align with the desired expectations of the resident and all decisions made are resident and family-centered.
- Ensures the plan of care is visible and adhered to by the interdisciplinary team. Conducts audits on care plan adherence
- Assist unit staff by coaching, mentoring, and monitoring the implementation of care plans.
- Ensures documentation for ongoing and acute issues is appropriate, complete, and timely.
- Provides support and guidance to staff in the identification of significant changes in resident needs and services.
- Identifies gaps and supports needed to support successful discharges for short-term residents.
- Performs education and return demonstration tactics to prepare residents and families before discharge.
- Understand the Nursing Practice Process including assessment, implementation, planning, and evaluation of resident care. Keeps current on nursing theory, research, medications, and legal issues related to resident-based services.
- Demonstrate LEAN leadership through coaching, inquiry root cause analysis, and removal of barriers to improved performance.
- Assist the Administrator in analyzing re-admission and ED visits for performance improvement.
- Attends and supports fall prevention meetings and strategies.
- Assists with other nursing duties and performance improvement projects as assigned (i.e. vaccine administration, Covid-19 outbreak testing, Kaizens/CIP event teams).

- LWMTMA- Trained Medication Assistant
- LWNLPN- Licensed Practical Nurse

Skills and Experience:
Required:
- 2-4-year Nursing degree
- 2 years of RN experience
- CPR certification
- MN RN license
Preferred:
- Minimum data set experience preferred
- Basic computer skills including Word, Excel, Outlook, and EMR (Electronic Medical Records)
- Non-violent crisis intervention and crucial conversation training to be completed within the first year of employment.
