Human Services Specialist
Direct Supervisor: Center Director
Admissions-Housing and Adult Daycare:
Facilitate referral calls and tours for potential assisted living and adult daycare clients.
Perform Intake Assessments.
Meet with the RN, Center Director, and ALM as appropriate to ensure that the potential client is appropriate for the program.
Verify transportation available if applicable.
Assure that ALL required documentation is received before admitting resident\daycare clients.
Update census and other lists with new admissions.
Ongoing/Annual Compliance for Programs:
As applicable to specific sites, maintain program paperwork, including CACFP, Senior Nutrition, and CDA.
Ensure VA and MA annual certifications are completed to ensure payment.
Support Center Director with all grant documentation as directed.
Manage changes with client days, funding sources, etc., and complete all required documentation.
Participate in Outreach:
Attend trade shows and community meetings as directed by the Center Director.
Assist with any marketing initiatives as directed.
Process Respite Care Requests and Maintain the Respite Calendar:
Process and scheduling temporary residents to include:
- Verifying with ALM and CM/DN if our program can meet the needs of the individual.
- Ensure all paperwork, payments, and medication review is completed prior to visit.
Support Clients/Families:
Assist families applying for benefits or needing referrals for County services.
Facilitate a monthly Caregiver support group as directed.
Communicate with family members on an ongoing basis regarding participation in the activity program,
positive/negative behavior issues, and any other non-medical issues.
Ensure quarterly care plan meetings are scheduled with resident and daycare families and participate in meetings along with ALM and/or RN.
Provide case management to clients and families as needed.
Support to Direct Service Staff
Assist with behaviorally challenging participants/residents.
Ensure staff using correct approaches when providing care.
Document issues in progress notes as needed.
Provide feedback on programming to meet the needs and interests of clients.
Provide guidance as needed to staff, helping them to maintain a sense of pride in their work,
consistently focusing on client needs, and maintaining a pleasant environment for clients.
Understand Winter Growth Privacy and HIPAA policies as well as all State, Local, and Agency policies.
Participate in interpreting these policies and philosophy, standards, and goals to staff and the community.
Be up to date on all regulations that pertain to assisted living: COMAR 10.07.14 (ALF), 10.09.54, (Home and Community-
Based Services Waiver). Be familiar with regulations that pertain to adult medical daycare to assist the Center Director with
compliance as needed: 10.09.61 (Daycare Waiver), 10.12.04 (Daycare) 10.09.07 (Medical Daycare-MA).
Minimum Physical Requirement: Able to operate a personal computer; concentrate for long periods while maintaining accuracy. Able to perform occasional physical activities including lifting, rotating, and assisting residents who are partially or dependent on transferring, including from wheelchairs, beds, or other sitting positions.
