Job Overview
NYS 1115 Medicaid Waiver
“Screener.” A contracted entity responsible for conducting a comprehensive screening of Member needs across a range of Health Related Social Needs (HRSN) domains using the Accountable Health Communities (AHC) Health Related Social Needs Screening Tool (or another comparable format, which has been approved by the SCN Lead Entity).
“Navigation.” The referral of a Member to providers of Enhanced HRSN Services, if eligible; or, the direction of a Member to HRSN services delivered by existing infrastructure, including pre-existing federal state, and local programs, if the Member is not eligible for Enhanced Health Related Social Needs Services.

Responsibilities
- Provide Screener/Naviagator assistance to individuals and families with NYS Medicaid insurance plans, helping to indentify community resources to address unmet needs to improve health outcomes.
- Willingness to assist community members with completing necessary paperwork and applications for insurance coverage or financial assistance.with by providing resouces within a clinic, hospital, healthcare or community setting without prejudice.
- Coordinate to identify if unmet needs are met by community resource partners.
- Maintain accurate records of Screener/Navigator interactions and progress.
- Serve as a community advocate to support the NYS 1115 Medicaid Waiver program address unmet needs of eligible members.
- Stay updated on changes in policies, regulations, and available community resources.
Experience
- Previous experience in providing community resources and advocacy is preferred.
- Strong interpersonal skills with the ability to communicate effectively with diverse populations.
- Familiarity with completing Medicaid health insurance terminology and processes is beneficial.
- Proficiency in using electronic health records (EHR) systems or similar software is an advantage.
- Ability to work independently as well as collaboratively within a team environment.
- A compassionate approach to patient care and a commitment to promoting health equity.
