Employment Type: Full-Time (40 hours per week)

Position Summary:
The Case Manager will play a pivotal role in the therapeutic process at Wolf Creek Academy by coordinating essential services, maintaining effective communication between families, medical providers, and the therapeutic team. This position ensures seamless integration of care while supporting the Director of Therapy in administrative and operational functions.
Key Responsibilities:
1. Medication Management:
- Oversee and track all student medication needs.
- Communicate regularly with the prescribing psychiatrist to address changes or concerns.
- Ensure accurate documentation of medication administration and compliance.
2. Parent Coordination:
- Organize, facilitate, and transport (school vans and gas will be provided) monthly student visits with parents, maintaining a schedule that ensures meaningful and productive interactions.
- Serve as a point of contact for parents to provide updates and address questions about their child’s progress.
- Organize, facilitate, and transport (school vans and gas will be provided) teens to their respective doctor's appointments. Communicate with staff and parents about upcoming doctor's appointments and follow up with parents.
3. Communication with Psychiatrist:
- Maintain regular communication with the psychiatrist to relay updates, concerns, and treatment plans.
- Assist the Director in Therapy in coordinating appointments and ensure all necessary information is provided for effective care.
4. Administrative Support:
- Assist the Director of Therapy with administrative tasks, such as scheduling, record-keeping, and maintaining compliance with therapeutic standards.
- Support planning and implementation of program initiatives as needed.
Qualifications:
Education: Bachelor’s degree in social work, psychology, counseling, or a related field is preferred, but not required.
Experience:
- Prior experience in a therapeutic, case management, or administrative role.
- Familiarity with adolescent mental health and family systems is highly desirable.
- CPR and First Aid Training is helpful
