LPN Full Time Night Shift Hospital based SNF

About the Role:

As a Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF) at Wooster Community Hospital, you will play a crucial role in providing high-quality patient care to residents with diverse health needs. Your primary responsibility will be to assist in the daily living activities of patients while ensuring their comfort and safety. You will collaborate closely with registered nurses and other healthcare professionals to develop and implement individualized care plans. This position requires a compassionate approach to nursing, as you will be a key support system for both patients and their families. Minimum Qualifications:

• Valid LPN license in the state of Ohio.
• Completion of an accredited practical nursing program.
• Current CPR and First Aid certification.

Preferred Qualifications:

• Experience working in a skilled nursing facility or long-term care setting.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor and record patients' vital signs and health status.
• Assist patients with daily activities such as bathing, dressing, and eating.
• Communicate effectively with patients, families, and healthcare team members regarding care plans and patient progress.
• Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
