About the Role:

The LPN (Licensed Practical Nurse) in the Part-time Day Shift Hospital Based Skilled Nursing Facility (SNF) plays a crucial role in delivering high-quality patient care within a dynamic healthcare environment. This position involves collaborating with a multidisciplinary team to assess, plan, implement, and evaluate patient care plans tailored to individual needs. The LPN will be responsible for monitoring patient health, administering medications, and providing education to patients and their families about health management. Minimum Qualifications:

• Current LPN license in the state of Ohio.

• Basic Life Support (BLS) certification.

• Certification in IV therapy.
• Knowledge of electronic health record (EHR) systems.
