Case Manager
Case manager needed for CBDS program with an arts focus. The Case Manager ensures adequate assessment and development of all program participants. The primary responsibilities of the Case Manager include tracking and maintaining program schedules, developing individual service plans, maintaining accurate case records, conducting satisfaction surveys, and providing follow-up services. They also assist with referrals, tracking individual goals, and exploring employment opportunities. Additionally, they ensure compliance with audit requirements and provide job coaching as needed.
Necessary skills for the Case Manager role include effective communication, maintaining good attendance, and excellent customer service. Data analytics and reporting skills are essential when working autonomously and as part of a team. Crisis intervention, de-escalation technique training and bilingual skills are preferred.
Qualifications
- Maintaining good attendance is required.

- Providing crisis intervention/de-escalation techniques and support as necessary.
- Fostering a positive work environment and staying flexible with work duties assigned.
- Able to work autonomously and as part of a team when needed
- Ability to perform data analytics and reporting
- Knowledge of Microsoft Office, PayCom system, HCSIS, Salesforce

Ability to communicate effectively to the workforce of English learners and those who use ASL may be needed.
