What are the responsibilities and job description for the Case Manager (AHT) position at Workforce Resource, Inc?

Lead youth that are 18-21 that have been human trafficked to their goals and assist them with healthy relationship, employment, education, housing, and community resources. Responsible for determining eligibility for Anti-Human Trafficking services based on TSBIRTS screening and for ongoing case management of youth have been previously screened by AHT liaison/other WRI Staff.

Answer calls on the Youth Services Hotline. The Hotline is to provide immediate response to Independent Living and Anti-Human Trafficking youth enrolled in the respective programs.

Career Planners assigned to the Hotline will work in collaboration with college staff from University of WI Systems to meet requirements of the grant.

Specific responsibilities include:

1. T-SBIRT screening of youth going into services at Workforce Resource:

  • Screen all youth under 21 for Trauma to determine qualification of AHT programing
  • Refer youth to services within the agency such as Support to Communities, FSET, TJ

2. Direct services for youth ages 18-21

o Providing primary direct services and case management for eligible young adults between the ages of 18-21.

o Streamlining access to and increasing utilization of housing, employment, positive social bonds, education, youth engagement in their community, positive youth development, and healthcare services, as needed and directed by each youth.

o Connecting youth pursuing postsecondary education to FAFSA and other postsecondary resources.

o Work toward co-enrollment of youth within the agency for career services and other necessary resources.

o Keep confidential records and case notes in an orderly manner.

o Expense out client requests to financial department in timely manner and deeming what reasonable and necessary for youth’s program goals.

3. Partnership with regional network of resources for youth

o Collaborate with community resources such as local service agencies, educational institutions, county and tribal welfare agencies, and local employers to address needs related to employment, education, housing, health, positive social bonds, finances and financial literacy, and other areas of self-sufficiency.

o Convene the network on a regular basis to encourage collaboration.

o Participate in/support the regional Youth Advisory Board and encourage youth to have meaningful input into program design.

4. Implement meetings for youth advocacy, healthy relationship, and life skills

  • Recruit youth on case load to have monthly meetings for No More Silence to have opportunities to share their experiences in a meaningful way to help with the direction of the program and work with State Department of Children & Families.
  • Teach curriculum on healthy relationships and life skills.

5. May be assigned to support Youth Services Hotline as a Representative. See Youth Services Hotline Representative job description.

Hotline Duties Include:

  • Answer Hotline

o Provide immediate emotional support and intervention to youth experiencing a personal crisis.

o Assess their situation, determine solutions, offer resources, connect with help, etc.; follow case management protocol per the respective program enrolled in.

  • Development of new programming and team collaboration with the university:

o Work with college staff from University of Wisconsin Systems (UWRF and MCW) for overall development of mental health and trauma response.

o Assist Program Manager and university staff with data collection

  • Intern Recruitment and field supervisor.

o Recruit local interns from local colleges to meet program needs.

o Proctor internships and complete college paperwork as necessary

o Make sure interns are meeting educational goals and work with program supervisor to reach goals of the program

QUALIFICATIONS: Skills in leadership, public speaking; knowledge of community resources; knowledge of regional business and the economy in the geographic area assigned; effective interpersonal and counseling skills; group facilitation and organizational skills; ability to keep accurate records; ability to travel. Have experience with trauma-informed care practices, minimum of 2 years of experience in working with adolescents or young adults

EDUCATION REQUIREMENTS: Bachelor's degree in Social Work, Psychology, Vocational Rehabilitation or related field from accredited four-year college or university or 2-3 years case management experience. Prefer experience working with youth ages 14 and up.

Service area: St. Croix, Pepin, Pierce and Dunn Counties.

Travel is required but some work is done via TEAMS.

Mileage is reimbursed.

May work from home but utilize WRI offices when meeting with clients. Office availability in Dunn County Job Center if preference is not to work from home.

Job Type: Full-time
