Work with individuals to seek and obtain community resources, employment, and housing. May assist with skill growth and refinement, budgeting, identifying community resources, career goal identification, and work readiness and job search training to secure community employment. Will also work collaboratively with community resource agencies and housing supports to include local landlords.

Key Roles:

1. Provide case management services in program and systems design that support navigating and assessing participant interests and service needs.

A. Identification and utilization of appropriate internal and external program services.

B. Monitoring of service plan progress.

C. Problem solving and resolution.

D. Assessing progress in participant attainment of program goals.

2. Assist in the identification and establishment of appropriate housing.

A. Conduct community outreach as advocate to identify appropriate housing opportunities for persons with disabilities and impart advocacy education to landlords and other members of the community with cooperative working relationships.

B. Develop landlord relationships to assure completeness of verification forms and other paperwork required to work collaboratively with those who utilize vouchers for housing support.

C. Attend home showings and site visits to determine appropriateness for individual or group needs prior to assisting with lease signing.

3. Assist with individual skill development and enhancement.

A. Observe and record participant activities and gauge participant skills.

B. Identify needs for support with accessing community resources.

C. Teach skills of developing budgets, setting up bank accounts, and managing money.

D. Attend appointments as appropriate to provide psychosocial support services.

4. Assist with vocational assessment, career planning, work readiness and job search training.

A. Assist in job seeking activities such as completing applications, preparing resumes, mock interviews, contacting and following up with prospective employers and utilizing employment strategies to achieve employment goals and objectives.

5. Maintain neat and accurate records and files.

A. Complete required documentation including case notes, training logs and progress notes in a neat, accurate and appropriate manner that meet all requirements and standards.

B. Report participant progress and site assessments using required case management systems.

C. Maintain participant and work/training site files in accordance with agency requirements.

6. Participate in multi-disciplinary Workforce Development Teams

A. Regularly communicate status of participants and work/training sites with team members.

B. Assist in program evaluation, quality improvement and other team required activities.

C. Attend staff meetings and participate in required training and professional development activities.

D. Perform other duties as assigned.

Qualifications: Excellent interpersonal, team work, problem solving, communication and computer skills, positive work ethic, ability to maintain accurate, neat, and complete records required. Knowledge and experience in vocational assessment, particularly with persons with disabilities, career planning, work readiness, local landlords, independent living skills, and housing supports desirable. Job requires a valid driver’s license and access to dependable transportation. May transport Clients as needed.

Education: Bachelor’s Degree, Field of Human Services

Must live within region to serve customer.
