Required:

Summary:
This position provides professional-level service coordination and case management for Health-Related Social Needs (HRSN), or Flex referred families and individuals who are experiencing homelessness and assist and advocate on behalf of participants to locate, secure, and retain affordable permanent housing. They will work with individuals or families who are experiencing homelessness, living in substandard conditions, or who need additional support to stabilize their housing.
Duties Include:
* Assist referred participants with housing applications including preparing/obtaining any needed documentation and tracking application status
* Assist in the development and implementation of policies and procedures related to HRSN
* Facilitate community-based events and convenings to promote access and raise awareness of HRSN services
* Keep participants informed of the status of their applications/referrals at each state of the process
* Assessing participants' housing needs and financial capabilities
* Providing participants with guidance on all housing-related concerns, including referrals to address landlord-tenant disputes and housing discrimination
* Assists participants in their housing search, including applying for rental units, making referrals to other housing location services, and accompanying participants to rental unit viewings as needed
* Communicates frequently with Cascade Health Alliance team members to provide updates on referred participants
* Attend weekly staff meetings with Cascade health Alliance team members
* Conduct regular home visits with HRSN participants
* Keep current with landlord laws, ordinances and regulations that impact housing
* Complete HRSN and Flex participant intake, including receipt and review of eligibility documents
* Develops working knowledge of community resources related to needs of individuals and families
* Conducts regular in-person post-placement visits, identifies, and mitigates risk to improve the likelihood of housing retention
* Monitor and evaluate the effectiveness of housing placements and services and adjusting plans as necessary to meet participants' evolving needs
* Develop rapport and communication with both property managers and participants experience homelessness
* Maintains and reports complete and accurate documentation of service objectives and outcomes
