Job Overview
Responsible for case management, coordination, and support for families enrolled in Relative Caregiver Program (RCP). Engages with families, completes assessment and service planning activities, ensures linkages to services and resources, advocates for families and assists with navigation through formal/informal systems. Completes home visits, crisis support, and parent training as needed. Participates in Child and Family Team Meetings and completes all required documentation.
Duties
- Receive program referrals and assess family program eligibility.
- Provide resource linkage, intervention, crisis stabilization, advocacy, and training.
- Coach families in problem solving, decision making, and action planning.
- Facilitate monthly peer and family support groups.
- Publicize program through outreach activities.
- Participate in community-based meetings and activities.
- Maintain an inventory of available community services and supports.
- Transport children and families, as necessary.
- Attend workshops, conferences and/or seminars to enhance professional growth.
- Maintain up-to-date and accurate case files, and electronic documentation.
- Complete all required documentation in a timely manner.
- Complete any other duties that may be assigned.
Education/Experience
Bachelor's Degree in social work or related humanities field and one year experience with youth and families. Full background clearance required, including TBI fingerprinting.
Skills
- Strong verbal, written, and active listening communication skills including communicating clearly, effectively, tactfully, and patiently with a diverse population.
- Strong interpersonal skills with the ability to build rapport and garner trust with others.
- Ability to learn new software applications; proficiency in the use of a PC in a Windows environment; operation of cellphone, scanner, and the Internet; proficiency in MS Office applications such as Word, PowerPoint, and Excel; as well as comfortability with Gmail.
- Experience and/or knowledge in crisis intervention, adverse childhood experiences (ACE’s) and the impact of trauma on children.
- Knowledge of best practice with families and the ability to coach and mentor adults and youth.
- Ability to facilitate training activities and support groups.
- Ability to plan and execute community outreach and awareness activities.
- Ability to prioritize tasks, follow through to completion and document work with families in a timely and accurate manner.
- Ability to work effectively with multiple families and/or tasks.
- Valid driver's license and current proof of insurance.
