This position is responsible for ensuring prompt and efficient bed management, assisting with staff resource utilization and Referral Line Management. Serves as a liaison to outside admitting physicians, outside facilities and the Emergency Department for patient referrals and direct admissions. This position takes a comprehensive assessment of each patient’s needs and develops nursing care plans. Manages the MDS processes in accordance with government regulations and policies with the focus on maintaining excellent care for patients.

MINIMUM QUALIFICATIONS :

EDUCATION, CERTIFICATION, AND / OR LICENSURE :

1. Current unencumbered licensure as a Licensed PracticalNurse withappropriate state board where services will be provided.

2. Obtain certification in Basic Life Support within 30 days of hire date.

1. Assists with the planning organization, development and coordination of the care planning process up on admission of all new patients.

2. Plays an active role in maintaining rapport with all departments to ensure a team effort is achieved in developing, implementing, and evaluating care plans for patients.

3. Assists in reviewing and evaluating the comprehensive assessment date and helps identify when and what additional assessments are triggered.

4. Completes and submits the MDS timely and accurately to ensure every MDS deadline is met.

5. Communicates effectively with residents and their families to ensure they are receiving excellent care, and their needs are being met throughout their length of stay.

6. Assists in maintaining the facility assessment calendar, coordinates the completion in of all MDS assessments, and ensures timely completion of assessments, care plans, and other required paperwork.

7. Check and resolve MDS errors.Ensuring MDS and Care plan compliance in all departments.

8. Manages the facility’s census by developing and maintaining positive relationships with hospitals, physician groups, community organizations and insurers.

9. Connect daily with potential referral sources including hospitals, physicians, insurers, case management companies and health care agencies.

10.Conduct facility tours for potential residents and families.

11. Conduct admission screenings of potential guests; determine level of care, services required, equipment needs, and insurance coverage as needed.

3. Ability to be flexible in response to changes in work volume, staff and scheduling changes.

4. Ability to work successfully under highly stressful conditions and capable of adapting to varying workloads and work assignments on a constant basis.
