Job Summary:
The Registered Nurse - Case Manager will provide direct and indirect nursing care to patients in their homes. This involves collaborating with the patient, family, and healthcare team to develop, implement, and evaluate nursing care according to agency and system policies.
Key Responsibilities:
- Develop, implement, and evaluate comprehensive nursing care plans for patients in their homes.
- Collaborate with the patient, family, and healthcare team to ensure timely and effective care delivery.
- Perform basic patient assessments, including vital signs, patient weights, and wound care.
- Monitor patient teaching and complete plan of treatment and recertification as per Medicare guidelines.
- Communicate patient care concerns or potential issues to the Administrative Staff and other interdisciplinary team members.
Requirements and Qualifications:
- One year of Home Care Nursing experience.
- Two years of prior nursing experience.
- Able to read and write legibly in English.
- Visual acuity within normal range.
- Ability to make sound, independent judgments based on scientific principles.
- Ability to collaborate with other multi-disciplinary team members.
This role requires extensive travel and flexibility to work weekends, holidays, and all shifts. The ideal candidate will have excellent communication and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
