What are the responsibilities and job description for the Social Worker position at YAI?
The ideal candidate will have a Bachelor's Degree in Social Work or a related field, with at least three years of social work experience under the supervision of a Master's Degree holder. They will also possess knowledge of California's Lanterman Act and its related service system, Title 17 and/or Title 22 regulations.
In this role, you will be responsible for completing intake assessments, developing and recording necessary information, and participating in transitional planning processes. You will also collaborate with interdisciplinary teams to ensure continuity of care and maintain accurate records of services provided.

Key Responsibilities:- Assists in the completion of assessments and plans for each person in the home
- Completes intake assessments during the referral process, reviewing current and historical records and documentation
- Participates in the transitional planning process working closely with the Qualified Behavior Modification Professional (QBMP)
- Maintains and appropriately documents each person's services records
- Collaborates with QBMP and interdisciplinary team to ensure continuity of care
- Acts as a liaison between the home and various parties on behalf of the people we support
- Bachelor's Degree in Social Work from an accredited program; or
- Bachelor's Degree in another Human Services field and at least three (3) years of social work experience under the supervision of a social worker with a Master's Degree;
- Holds (or obtains through employer-provided training) CPR/First Aid, Ukeru and CPI certifications and recertifies annually;
- Knowledge of California's Lanterman Act and its related service system, Title 17 and/or Title 22 regulations;
- Highly organized and flexible with ability to manage time and work autonomously;
- Exceptional verbal and written communication, interpersonal and presentation/training skills;
- Proficiency in technical systems, such as Behavior Agent, Workday, Microsoft Teams, etc.
