Position Highlights:
$31.46-$38.66/hour DOE with the ability to go higher for highly experienced candidates
$5,000 hiring bonus structure:
At Hire: $1,500
At 90 Days (3 months): $1,500
At 180 days (6 months): $2,000
Bonus will be pro-rated based on Full Time Equivalency (FTE)

What You'll Do:
Obtain complete history and conduct chart review, ensuring inclusion of all reports needed
Perform baseline physical assessment and expand assessment based on initial findings
Collaborate with healthcare providers to develop and implement plans of care
Coordinate and plan interagency referrals as directed by the Provider or Registered Nurse (RN)
Assist Providers in diagnostic procedures and treatments
Provide education to the patient and family/caregivers, taking into consideration age, development, culture, language, and available resources
Coordinate plan of care and other resources
Evaluate care based on assessments and Physician orders and initiate a follow-up care plan
Assist in managing medications based on purpose, side effects, and drug interactions
Collaborate with ancillary staff to process patients based on Providers' schedule
Oversee subordinate nursing staff's provision of patient care
Plan and organize clinical supplies, equipment, and furnishings to maintain a safe patient environment
Perform other duties as assigned
Qualifications:
Graduation from an accredited Licensed Practical Nurse (LPN) program
One year's experience as an LPN is preferred
OR LPN License
Cardiopulmonary Resuscitation (CPR) certification within 90 days of hire
Bilingual (English/Spanish) preferred at level 10 for bilingual differential pay
Understanding of LPN concepts, medication effects, and community resources
Strong communication skills, attention to detail, and accuracy
Ability to assess patient needs, prioritize tasks, and collaborate in a team
Basic computer skills with Microsoft programs, proficiency in EMR a plus
