About the Role
The Registered Nurse (RN) is responsible for assessing patients' health needs, developing individualized care plans, and implementing evidence-based interventions to promote optimal health outcomes.
Key responsibilities include:
- Collecting and analyzing data to identify patient needs and priorities
- Developing and implementing comprehensive care plans that address physical, emotional, and social needs
- Collaborating with interdisciplinary teams to ensure seamless care transitions and coordinate services
- Providing education and support to patients and families to promote healthy behaviors and self-care
Requirements
To be successful in this role, you will need:
- A degree from an accredited nursing program
- Current licensure as a Registered Nurse in the State of Connecticut
- Basic Life Support (BLS) certification
- 1 year of acute care or ambulatory RN experience
- Prior experience in oncology and/or ambulatory settings is preferred
