About the Role
We are seeking an experienced Ambulatory Care Registered Nurse - OR to join our team at Yale New Haven Health. The successful candidate will have a strong background in urology nursing and excellent communication skills.
As a member of our healthcare team, you will be responsible for assessing patients' physical and psychosocial conditions, providing education, and assisting with various intra-office procedures.
You will work closely with our interdisciplinary team to ensure the delivery of high-quality patient care in the ambulatory setting.
- Key Responsibilities:
- Assess patients' physical and psychosocial conditions on an ongoing basis throughout the continuum of care
- Develop and implement individualized plans of care to meet the unique needs of each patient
- Collaborate with healthcare professionals to coordinate care and promote effective communication
- Maintain accurate and up-to-date patient information, including health history, surgical history, medications, allergies, and preferred laboratory and pharmacy
