POSITION TITLE: Healthy Homes Coordinator

RESPONSIBLE TO: Energy Services Director

RESPONSIBLE FOR

This position is a non-exempt position responsible for managing all aspects of development, implementation and documentation for the county-wide Healthy Homes Initiative and appropriate Medicaid Health Related Social Needs’ projects. The Coordinator will perform home assessments and inspections and identify situations that threaten or impact health and safety and oversee the corrective interventions. The position provides energy education and departmental support for weatherization project management and sub-contractors management.

ESSENTIAL DUTIES:

  • Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses.
  • Assist the Energy Services Director in oversight of the program budget, expenses, change orders and ensure grant reporting is accurate and timely.
  • Perform accurate home assessments by thorough examination of building components for the identification of trips, slips, falls, other safety concerns, indoor air quality and potential problems in structure and integrity of structures.
  • Conduct tests on homes and appliances including pressure diagnostic, combustion appliances and furnaces for safety and efficiency.
  • Manage & coordinate healthy home/weatherization projects (client cases) including contractor oversight, documenting data points and outcomes, developing service plans, providing information and referrals, maintaining data/records and ensuring compliance with Lead Safe Weatherization requirements.
  • Manage & coordinate Medicaid Health Related Social Needs’ projects including medically necessary home accessibility modifications and medically necessary home remediation including contractor oversight, documenting data points and outcomes, developing service plans, providing information and referrals, maintaining data/records and ensuring compliance with Lead Safe Weatherization requirements.
  • Maintain contracts, records and files for program and fiscal requirements in a timely manner. Prepare reports, input data and assist with energy audits as required for program, fiscal and grant requirements.
  • Input data in agency and state computerized programs to generate timely reports including but not limited to weekly contractor/vendor job estimates, cost accounting, energy education reports, and month ending reports.
  • Assist Energy Services Director in the assembly of information, preparing and tracking correspondence, grant applications, contractor/vendor meetings and/or special projects.
  • Expand agency network and fund development opportunities through community and professional contacts, training, and involvement in collaborations and cooperative activities with other agencies, groups, or organizations. Including occasional out of town travel.
  • Other duties as assigned.

KNOWLEDGE, SKILLS, & ABILITIES:

· Associate’s degree (A.A.) and/or related experience and training

· Ability to complete and maintain state certifications

· Ability to calculate figures and apply concepts of basic algebra and geometry

· Skill in verbal & written communications, problem solving, attention to detail and team work

· Ability to meet the physical demands and perform essential duties

· Strong interpersonal skills for connecting and working with persons from diverse backgrounds (including, but not limited to, ethnic, religious, physical and mental abilities, etc.)

· Knowledge of construction practices & ability to organize detail-oriented projects and work independently on multiple projects in a timely fashion

· A familiarity with resources in Yamhill County

· Recognition of appropriate boundaries and the ability to implement them and maintain client confidentiality

· Ability to problem solve and work effectively in stressful or crisis situations

· Bilingual- (English and Spanish) and bi-cultural experience and abilities preferred

· Commitment to work as an essential part of a team

· Ability to be flexible in work schedule as needed (evenings, weekends, & out of town travel)

· Ability to pass drug/alcohol & background check and insurance check of driving record

PHYSICAL DEMANDS

The physical demands described represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job the employee will be required to sit, stand, kneel, walk or bend for extended periods of time. The employee is frequently required to reach with arms, hands; climb or balance; stoop, kneel, crouch, crawl in tight/confined spaces or work from ladders; walk on steep roofs, wet roofs; and work in all types of weather. Must frequently lift and/or remove up to 40 pounds and occasionally lift up to 75 pounds.

Job Type: Full-time

Pay: From $24.67 per hour

Expected hours: 40 per week
