POSITION TITLE: Veterans Case Manager
RESPONSIBLE TO: Housing Stabilization Director
RESPONSIBLE FOR:
This is a full-time, non-exempt position providing case management to individuals and families who are homeless or at risk of homelessness, including veteran households enrolled in the Supportive Services for Veteran Families Program. It involves analysis of their situation, application of possible services available to help, and on-going goal-setting and case management to ensure the individual or family is stabilized.
ESSENTIAL DUTIES:
1. Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses.
2. Assess client needs and provide referrals to agency programs or to other community resources as appropriate.
3. Provide case management for clients in YCAP’s SSVF and Turnkey program including activities such as applications to services, goal setting, advocacy, and referrals to partner agency services. This will include driving to client locations on a daily basis within Yamhill County.
4. Participate in targeted and street outreach efforts to identify homeless veterans.
5. Provide timely report, input data, and ensure all files and reports are accurate and grant standards are met. Assist with data for monitoring site visits.
6. Participate in bi-weekly SSVF conference calls.
7. Assist with agency activities around homelessness including such things as the PIT Homeless Count, the Yamhill County Veteran Stand Down, and Community Connect.
8. Collaborate with other agencies, businesses, landlords and volunteers to ensure that agencies’ programs coordinate to provide the best use of resources. This will include community outreach on behalf of the program.
9. Requires completion of specialized training that meets Veterans program requirements, including but not limited to: crisis intervention and de-escalation, human trafficking, suicide prevention, harm-reduction, and trauma-informed care.
10. Other duties as assigned.
KNOWLEDGE, SKILLS, & ABILITIES
- BSW and/or equivalent experience
- Knowledge of computers, software programs and databases
- Skill in verbal and written communications
- Skill in problem solving and team work
- Ability to maintain confidentiality of client and organizational information
- Ability to work independently on multiple projects in a timely fashion
- Ability to work with diverse people, organizations and situations
- Ability to work with a flexible schedule
- Ability to pass background, driver and drug screen
- Veteran status is helpful, but not required.
PHYSICAL DEMANDS
The physical demands described represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job the employee will be required to sit, stand, or walk extended periods of time and will occasionally lift up to 25 pounds.

Pay: From $24.67 per hour
