Duties:
- Provide one-on-one support to clients, including crisis intervention, counseling, and advocacy.
- Conduct thorough assessments of client needs, including those related to behavioral health, addiction, and child welfare.
- Develop and implement personalized plans to address client needs, including referrals to community resources and medical services.
- Collaborate with other professionals, such as healthcare providers, addiction counselors, and childcare workers, to ensure comprehensive support for clients.
- Monitor client progress, identifying areas of success and areas for improvement, and adjusting plans accordingly.
- Facilitate group sessions, workshops, and support groups to promote skill-building, social connection, and community engagement.
- Engage in workforce development initiatives to help clients achieve their employment goals.
- Ensure compliance with relevant regulations, including Medicare guidelines.
- Maintain accurate and timely records of client interactions, assessments, and progress.
Requirements:
- A bachelor's degree in social work, counseling, or a related field.
- At least two years of experience in case management, crisis management, or a related field.
- Strong knowledge of behavioral health, addiction counseling, and child welfare principles and practices.
- Ability to work effectively in high-pressure situations, including crisis intervention and emergency situations.
- Excellent communication and interpersonal skills, with the ability to build trust with clients and colleagues.
- Proficiency with Microsoft Office and electronic medical records (EMRs).
- Experience with Medicare and other healthcare programs.
- Certification in crisis intervention, addiction counseling, or a related field is preferred.
