Social Worker Responsibilities
- Participate in an interdisciplinary team approach to better serve the needs of the center population.
- Provide for therapeutic interventions to help residents cope with the social and psychological aspects of their illnesses, impairments, or disabilities as well as their feelings about institutionalization and separation from family, community, physical and emotional loss.
- Maintain written documentation in the resident medical record per the company policy.
. Perform an initial evaluation/assessment of each resident’s psychosocial needs. Written documentation at least quarterly required for each resident.
- Perform other job-related duties as they become required.
Social Worker Qualifications
- Possess a CSW or LSW within the state of operations (PA)
- Must possess at a minimum, a bachelors’ degree in social work or a bachelors’ degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
- Must have, at a minimum, one (1) year supervised social work experience in a health care setting working directly with residents.
- Experience in the long-term care setting or the drive to work in a challenging environment. 1-year Social Work experience.
- Must have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to manage residents based on maturity level they are currently functioning.
