Job Title: Children's Mental Health Targeted Case Manager, Senior
FLSA Status: Exempt
Reporting to: Supervisor, Case Management
Location: Richfield or Brooklyn Center
Purpose of the Position:
Case management provides services that are coordinated with community support services and are designed to help children with severe emotional disturbances and their families obtain needed mental health, social, educational, health, vocational, recreational, and related services in the areas of volunteerism, advocacy, transportation, and legal services. Case management includes assisting in obtaining a comprehensive diagnostic assessment if needed, developing a functional assessment, developing an individual family community support plan, and assisting the child and the child's family in obtaining needed services by coordination with other agencies and assuring continuity of care. Case managers must assess and reassess the delivery, appropriateness, and effectiveness of services over time.
Goals and Outcomes:

Average minimum of 14 active clients seen per month.

Provide outcomes that demonstrate a positive relationship has been built, risk factors have decreased and that positive growth has occurred or is in progress.

Ensure that outcomes reporting for caseload is 100% completed in the required timeframe.

Ensure that 100% of outlined case management IFCSPs and functional assessments are completed within timeframe.

All paperwork is done in accordance with organizational, DHS, insurance, and county guidelines.

100% of authorizations are up to date.
-
Be eligible for incentive based pay based on productivity and quality of service.
- Complete additional tasks supporting program operation as assigned.
Minimum Qualifications:
Education:
Bachelor's degree in one of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university.
Experience Required:

Employed as a Case Manager continuously for one or more years at Youable Emotional Health.

Two years of experience a Case Manager.
Knowledge, Skills and Abilities:

Proficient knowledge and understanding of mental health issues and resources.

General knowledge and understanding of case management services.

Proven ability to establish and maintain positive rapport with a wide variety of clients and professionals.

Demonstrated ability to identify and resolve problems independently in a variety of arenas with both clients and professionals within the community.

Demonstrated excellent verbal and written communication skills.
-
Demonstrated ability to collaborate and utilize the skills of program team and other staff.

Proven ability to utilize client strengths to promote positive change.

Proficient computer skills in Word and Excel programs.

High degree of organization and ability to work independently.

Ability to manage time efficiently in order to ensure paperwork and billing are on time and completed correctly.
Working Condition/Physical demands:

Exposure to fragrances and other environmental elements that may affect a chemical sensitivity.

Local travel within the metro area.

Availability to work at least two evenings a week.

Must have working vehicle and auto insurance

Visits and services provided in a variety of arenas, including in-home, in the community and in schools

Interact with a range of personalities and diverse individuals.

Ability to lift up to 25 lbs.
