Project Overview
The SF Justice Initiative is a multi-pronged strategy to disrupt the school-to-prison pipeline for middle schoolers by supporting them in their schools and preventing them from entering the juvenile delinquency system. This initiative includes a collaborative partnership with Coleman Advocates for Children and Youth, Young Community Developers (YCD), Legal Services for Children, Disability Rights California, and Youth Law Center.
The School Crisis Response Program Initiative is a collaborative that plays a crucial role in fostering a safe and supportive environment within SFUSD by addressing incidents of violence, providing crisis intervention, and implementing prevention strategies. This Initiative includes a partnership with Samoan Community Development Center, Wraparound Project, Roadmap to Peace, Tailormade, Community Youth Center SF, DCYF, JPD and SFUSD.
POSITION SUMMARY
The SSMP Life Coach will collaborate with the SF Justice Initiative provides mentorship, care management, advocacy, and support to middle school youth who experience truancy, behavioral, and/or disciplinary issues in select schools with historically high rates of suspension. The position will offer various levels of support to students and faculty. VI’s provide assistance in various situations that may occur at school and outside of school. They will also work in communities and collaborate with other community-based organizations and entities that assist in supporting youth and young adults. They will also support in relaying critical intelligence and information to the SVIP extended team.
RESPONSIBILITIES:
- Provide mentoring/care coordination support to SFUSD students during normal school hours
- Conduct assessments of youth to identify their needs and goals to develop a care plan
- Work collaboratively within the team to develop interventions and supports for the youth and their families.
- Develop and facilitate care planning with specific goals related to their academic and social-emotional learning and development (SEL/D).
- Maintain ongoing caseload and consistent and timely written and electronic documentation of contacts with youth, their families, and community partners
- Make and maintain contact with school officials, counselors, and other service providers/adult allies associated with the youth.
- Collaborate with parents to provide guidance, resources, and support, serving as an advocate to address their needs and ensure positive outcomes for their children.
- Conduct weekly home, community, and school visits for participants.
- Provide one-on-one and group sessions for youth
- Provide classroom observation and support
- Provide Support during IEP Meetings, Expulsion Hearings and Reentry planning
- Provide check-in with student and guardians
- Prepare reports for DCYF-CMS and weekly citywide weekly coordination meetings
- Participate in weekly youth services and supports meeting
- Connect youth to additional services as identified by the School Crisis Support Team
- All other duties as assigned
WHAT YOU WILL BRING
A successful candidate will possess:
- B.A./B.S. degree in a related field and at least two years of relevant experience preferred or Associates degree and four years of related experience. Candidates with 5 or more years' relevant experience in lieu of a degree will be considered.
- Clearance of TB test and fingerprinting are required.
- Must not be on active probation or parole
- Ability to work between the hours of 8:00 am and 5:00 pm at SFUSD school sites
- Ability to travel to all SFUSD school sites as required
- Excellent cross-cultural communication skills and the ability and willingness to work with economically and culturally diverse clientele from various professional and ethnic backgrounds.
- Excellent interpersonal and communication skills, written communications, patience, diplomacy, and high level of customer service skills.

ADDITIONAL QUALIFICATIONS:
To best mitigate the spread of disease and protect our staff, it is a condition of employment that all employees be fully vaccinated against COVID-19. Because YCD prioritizes the health and well-being of employees, proof of vaccination will be required upon hire and be kept on file for each employee.
