Case Manager - Basic Center

Summary: To be a key member of the program team participating in the care, behavioral intervention and support of at risk youth.  Monitor, develop and coordinate all aspects of the clients program participation, including but not limited to Educational, Vocational, Social, Financial, Recreational and interpersonal Development.  Implement program goals and behavioral interventions as set by the Program Director/Manager. Provide documentation to support program participation.  This position works in collaboration with Program Personnel and Community Resources to provide a successful transition to self-sufficiency and prevent long-term dependence on the social service system.

 Essential Duties and Responsibilities include the following.  Other duties may be assigned.

  • Provide referral, advocacy, crisis intervention and counseling services to youth.
  • Liaison between Youth Continuum and DCF social workers and/or legal guardians.
  • Provide Case Management services, including liaison with community resources.
  • Provide supervision to ensure the safety and stability of program participants.
  • Monitor program participants via announced and unannounced visits.
  • Provide on-call coverage to program youth, 24 hrs/7 days.
  • Participate in facilitating monthly groups with a focus on life skills development.
  • Develop a written Individual Independent Living plan with program participants outlining individualized goals including, but not limited to, employment, education, housing, budgeting and discharge planning.
  • Monitor and document progress on goals.
  • Assist with seeking and obtaining appropriate, safe and permanent housing prior to discharge.
  • Establish a network of landlords within the service area.
  • Assist with seeking and obtaining appropriate, safe and permanent housing prior to discharge.
  • Assist with medical needs.
  • Assist in developing and monitoring client budget.
  • Maintain/oversee all client related documents and files; maintain confidentiality.
  • Complete intake procedures with clients.
  • Participate in on going agency training and staff meetings.
  • Provide coverage at other programs in the event of an agency emergency.
  • Maintain appropriate boundaries with program participants.
  • Complete funding source reports.
  • Complete case notes on assigned clientele.
  • When appropriate, provide after care services.
  • Monitor client attendance and progress in academic and/or vocational programs.
  • Perform other duties as assigned, i.e. clean vacated apartments.

Qualifications

EDUCATION and/or EXPERIENCE: Bachelor’s degree is required in a related field.  Must have two to three years experience working with at risk youth, homeless youth, foster care youth, and/or young parents in a transitional and/or independent living environment.

LANGUAGE SKILLS: Ability to communicate clearly and appropriately in English, both written and verbal. Computer literate.

MATHEMATICAL SKILLS: Ability to evaluate, develop and monitor client’s budgetary needs.

REASONING ABILITIES: 

  • Ability to make sound decisions.
  • Ability to understand role differentiation between staff and client.
  • Ability to use effective time management.
  • Ability to bring issues to the attention of the Program Coordinator.
  • Ability to use judgment when in the community to keep self and clients safe from harm.

CERTIFICATES, LICENSES, REGISTRATIONS, and OTHER REQUIREMENTS

  • First Aid/CPR
  • Therapeutic Crisis Intervention
  • A current CT Driver’s License
  • Maintain a clear driving record.
  • Maintain a clear criminal record.
  • Maintain a clear DCF abuse and neglect record.
  • Annual TB screening.

  • Ability to provide physical intervention to youth in crisis utilizing Therapeutic Crisis Intervention.
  • Ability to provide assistance to clients in activities of daily living.
  • Ability to supervise and monitor client moving from one living arrangement to another.
  • Ability to endure long periods of sitting, standing, walking.

  • Ability to work with and be understanding of the diversity of staff and clientele.
  • Ability to work in an environment with frequent interruptions and moderate noise level.
