STREET OUTREACH CASE MANAGER
Summary:
Assists in the coordination and development of outreach and case management services; works collaboratively with a team serving a diverse population of high risk/homeless youth. Provides positive interactions with clients within the framework of the Street Outreach Program and the agency.
Essential Duties and Responsibilities:
- Coordinate and provide street outreach to homeless and runaway youth in the greater New Haven area.
- Work as part of a team to provide positive interactions and appropriate services to clients.
- Interact with a variety of public and private agencies and organizations, referral sources and others as directed by the needs of the program.
- Interact with law enforcement agencies.
- Represent program to funders, community, public/private groups.
- Keep daily and weekly demographic records.
- Create monthly calendar of outreach activities.
- Assist with outreach quarterly reports.
- Develop in-house outreach activities.
- Provide direct coverage to other agency programs in the event of an agency emergency.
Qualifications
QUALIFICATIONS:

EDUCATION and/or EXPERIENCE:
- Bachelor’s Degree in Human Services required or must be willing to pursue and obtain Bachelor’s Degree.
- 1-2 years experience with youth outreach work and at-risk/homeless youth essential.
LANGUAGE SKILLS:
Ability to communicate clearly and appropriately in English, both verbal and written.
Computer literate.
MATHEMATICAL SKILLS:
Ability to apply general mathematical concepts to practical situations.
REASONING ABILITIES:
Ability to problem solve with staff and youth.
Ability to bring issues to the attention of the Program Manager.
Ability to exercise appropriate authority and make sound judgment when needed.
CERTIFICATES, LICENSES, REGISTRATIONS, and OTHER REQUIREMENTS:
First Aid/CPR Certification
Therapeutic Crisis Intervention
Current Connecticut Driver’s License with a Public Service endorsement (PSL).
Maintain a clear driving record
Maintain a clear criminal record
Maintain a clear DCF abuse and neglect record
Bi-annual physical; Annual TB screening.
