Position Overview
The Transitional Services Case Manager oversees the Touchstone Village (TSV), a 20-unit transitional living program and the Maternity Independent Living and Life Skills (MILLS) program, a 9 unit tiny home community. This role delivers life skills training, and ensures policy compliance across programs. The case manager supports residents' academic, vocational, medical, and behavioral needs, facilitates coordination with community partners, and maintains comprehensive documentation for all residents. This position collaborates closely with the Housing Clinical Manager to ensure effective program delivery and positive client outcomes.

Position Duties and Responsibilities

- Oversee daily operations, safety, and security of both TSV and MILLS units.
- Provide life skills training using agency-approved curricula through group and individual sessions.
- Facilitate completion of IL assessments (Casey Life Skills, PAYA) and tabulate scores.
- Develop and review individualized Service Plans in collaboration with residents and community partners.
- Monitor resident progress and promote advancement through the program.
- Coordinate services such as academic, medical, psychiatric, career development, and social needs.
- Ensure all documentation (progress notes, service plans, incident reports, etc.) is complete and timely.
- Lead and document emergency drills and health/safety inspections as required.
- Monitor resident behavior and coordinate Safety Net meetings as needed.
- Transport residents to appointments and support admissions by completing intake paperwork and database entry.
- Provide outreach to stakeholders and represent the program in the community.
- Meet all personal and professional training requirements.
- Perform other duties as assigned to support the mission and operation of the programs.
Education and Experience
- An advanced degree (Master’s or higher) in Social Work or a related Human Services field, experience delivering youth services, and supervisory experience;
- OR
- A Bachelor’s degree in Social Work or a related Human Services field, at least two (2) years of direct service experience with a comparable population, and three (3) years of supervisory experience.
- A valid Florida Driver’s License and safe driving record is required.
Mathematical Skills and Reasoning Ability
Basic and/or advanced computational skills may be necessary.
Physical Demands
This is a position which requires walking, interacting, monitoring and engaging youth in various positive activities and diffusing challenging situations that will occur. Little to moderate lifting of less than 25 lbs. may be required.
Work Environment
While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
