The Basic Center Youth Advocate is a full-time position that is responsible for case managing Basic Center MELD and Bridge shelter residents. Services are guided by State and Federal policies as well as Youth Services Network, Inc.
Job Summary:
We seek a Bachelor's level applicant to provide case management and shelter services to identified youth. In addition, the incumbent will provide community outreach by providing information to our community regarding our services. Case management services may include school advocacy, family support, linkage to community resources, treatment planning, aftercare, and more. The candidate is expected to be trauma-informed and prepared to provide services to families and youth in crisis.
Requirements:
- Minimum of a Bachelor’s Degree in a human service-related field.
- Minimum of one (1) year experience working with children/adolescents.
- Valid driver’s license, proof of automobile liability insurance, and access to an automobile
to provide transportation to clients.

- Ability to exercise discretion and ensure confidentiality.
- Capable of handling high-stress situations.
- Works well in a team environment.
Duties/Responsibilities:
1. Complete a thorough intake assessment in a timely manner per grant requirements.
2. Provide crisis intervention, reunification, and stabilization services to clients and their
families.
3. Provide ongoing case management services, including family support, school advocacy,
linkage to community agencies, treatment planning, discharge planning, and more.
4. Develop a goal plan with residents and support residents in the implementation of the goal plan.
5. Document all contacts with or on behalf of clients and maintain updated case files.
6. Input client information into the RHY-HMIS system.
7. Provide community outreach to schools, police stations, fire departments, and other
partnering entities.
8. Complete required assessments and reports as needed.
9. Transport youth to school and alternative living arrangements when necessary.
10. Coordinate with CCBYS case worker to create and implement an aftercare plan.
11. Make referrals to YSN programs and/or community resources as needed.
12. Attend all scheduled staff meetings, agency meetings, in-services, and trainings.
13. Maintain confidentiality of all client information.
14. Perform other related duties as assigned.
Job Type: Full-time
Pay: $45,000.00 - $52,000.00 per year
