What are the responsibilities and job description for the Intensive In-Home Team Member position at Youth Unlimited?
Scope of Essential Responsibilities:
1. Implements Person Centered Plan objectives for each client on a daily basis.
2. Works with team leader to meet required minimum staffing requirements.
3. Works with client and family in client’s home 60% of time and complies with program requirements and agency policy and procedures.
4. Demonstrates a proactive approach to problem-solving.
5. Completes required professional development.
6. Establishes and maintains professional working relationship with parents, schools, courts, social workers, case managers and all other parties involved in the treatment process of each client.
7. Participates in all Performance and Quality Improvement activities.
8. Communicates all maintenance, financial and staffing needs to Program Director.
9. Accurately completes all documentation regarding client treatment and progress.
10. Administers all agency programs and activities in compliance with agency policies.
11. Participate in supervision as required by 10A NCAC 276.0104.
12. Performs other duties as requested.
13. Demonstrates a working knowledge of COA standards.
14. Reviews and participates in program outcome and performance improvement
process.
Work and Skill Requirements:
- Meet established criteria for privileging as a Associate or Qualified Professional
- One – three years experience working with population preferred
- Prior experience in Crisis Intervention preferred
- Ability to pass CPR, First Aid, Seizure Management, NCI
- Ability to follow direction as evidenced by being a contributing partner within a multidisciplinary team, engage in clinical supervision, implement and track clinical and outcome measures, practice integrity.
- Good organizational skills
- Ability to be sensitive to client cultural and social economic characteristics
Other Requirements:
- No criminal convictions relating to violent crimes, abuse, neglect, exploitation, drug use or drug trafficking
- Valid NC driver license and ability to pass all background checks
Education Requirements
- A graduate of a college or university with a Masters degree in a human service field and has one year of full-time, post-graduate degree accumulated mh/dd/sa experience with the population served; or
- A graduate of a college or university with a bachelor’s degree in a human service field and has two years of full-time accumulated mh/dd/sa experience with the population
- A graduate of a college or university with a bachelor’s degree in a field other than human services and has four years of full-time, post bachelor’s degree accumulated mh/dd/sa experience with the populations served.
- Must complete 20 hours of Intensive In-home training within 90 days of employment.
- Be a graduate of a 4 year university in a human service related field with zero experience (Associate Professional)
Physical Requirements
- Be of good physical and mental health, with no indication of active tuberculosis, communicable diseases or any other health condition that may pose a threat to clients.

Pay: $40,000.00 - $48,000.00 per year
