Overview
We are seeking a dedicated and compassionate Qualified Professional to join our team. This role is essential in providing high-quality care and support to individuals in various settings, including nursing homes, assisted living facilities, and home community care. The ideal candidate will have experience in supervising staff and managing care plans for individuals with developmental disabilities and those requiring long-term care.
Duties
- Supervise and coordinate the activities of staff members to ensure the delivery of effective care.
- Develop, implement, and manage individualized care plans tailored to meet the unique needs of each client.
- Collaborate with healthcare professionals, social workers, and family members to enhance the quality of life for residents.
- Provide guidance and support to staff in delivering compassionate care within nursing homes and assisted living environments.
- Monitor the progress of clients and adjust care plans as necessary to ensure optimal outcomes.
- Maintain accurate records of client progress, incidents, and any changes in health status.
- Advocate for clients’ needs and rights within the community and healthcare systems.
- Ensure compliance with all regulatory requirements related to long-term care and home care services.
Qualifications
- Proven experience in supervising staff within a nursing home or assisted living setting is preferred.
- Background in social work or related fields is highly desirable.
- Strong understanding of care plans and their implementation for individuals with developmental disabilities.
- Experience working in home community care or long-term care environments is a plus.
- Excellent communication skills with the ability to work collaboratively with diverse teams.
