Reports to: Housing Program Director

Salary: $45,760-$48,880/Year

Hours: Full time

The Women's Housing Programs aim to help homeless women find and thrive in safe, independent housing. Services are client-centered, trauma-informed, and empowerment-based. The YWCA respects residents as individuals and tailors housing plans and services to their unique needs. The YWCA serves adult women and transgender individuals who identify as women or non-binary. The YWCA strives to promote equity and provide relevant services to individuals from all walks of life.

Position Summary

Case Managers are responsible for the operations of the various YWCA Housing Programs, including but not limited to; applicant screening, admissions, case management, skill building, conflict resolution, coordinated referrals to medical and social services, assisting residents with physical needs, maintaining records, completing program reports, and maintaining a sanitary environment. This position also establishes and maintains relationships with service providers in the community. The Case Manager will need to physically turn over rooms, which can include moving furniture, cleaning floors, spraying for pests, and deep cleaning.

As part of the YWCA staff, there are group projects where all hands are on deck. This may include clearing abandoned rooms or setting up a new unit.

Responsibilities and Duties:

· Interview prospective residents referred to the YWCA through the coordinated entry system at the Whatcom Homeless Service Center.

· Contact Homeless Service Center when vacancies occur and submit a referral request.

· Coordinate admissions, including room assignments, completion of paperwork and new resident orientation. Read over and agree to program guidelines.

· Assist residents with their housing plan including addressing any barriers to housing. Provide advocacy and problem solving to obtain and maintain permanent housing. Provide information about community resources that can assist in this process.

· Be able to advocate and assist with medical appointments, including keeping track of times/dates, providing transportation if appropriate, attending appointments, and assisting with understanding and remembering client medical issues.

· May need to assist with medication, but NO care required that includes assisting with ADL’s (Activities of Daily Living) this includes bathing, feeding, waking etc.

· Provide on-going case management, as well as detailed case notes, with residents related to substance abuse, assisted living, behavioral health, legal issues, financial management, support groups, education, and conflict resolution.

· HMIS data entry – must complete mandatory HMIS training through Department of Commerce. Maintain accurate HMIS data on YWCA residents at intake, during stay, and at exit.

· Foundational Community Supports billing – Must complete FCS training and maintain records to bill for housing and employment services.

· Maintain accurate records, statistics and documentation regarding occupancy, emergency shelter bed-nights, and other relevant data as required by contracts and outcome reports.

· Assign and distribute weekly chore duties schedules and supplies. Follow up as needed.

· Respond to emergencies, including carrying after hours cell phone and/or coming to the YWCA outside of work hours as needed.

· Address resident conflicts through meditation and problem solving.

· Monitor resident’s compliance with program guidelines.

· Prepare rental references and acknowledgement of program participation as needed.

· Maintain confidentiality except in circumstances required by law.

· Participate in staff meetings and ongoing trainings as required by funding.

· Work as a team, filling in for others as needed.

· Participate and represent the YWCA in community meetings such as Whatcom County Coalition to End Homelessness.

· Must be willing to use personal vehicle to transport clients to necessary locations (doctor appointments, etc.)

· Willing to be on-call in the case of lockouts or other emergencies one out of every five weeks.

· Other duties as assigned.

Qualifications:

Education:

· BA in Human Services or related field or an equivalent education and work history.

Work experience:

· Two years’ experience working with vulnerable populations. We encourage people with lived experience to apply.

Skills and Attributes:

· Strong organizational skills

· Must have the ability to work both independently and with a team

· Must have the ability to adapt to frequent change and to handle stressful situations effectively, including use of de-escalation techniques.

· A positive outlook with highly effective problem-solving and conflict resolution skills.

· Strong written and verbal communication skills.

· Proficiency with Microsoft Office programs and data entry.

· Understands issues of oppression, homelessness, violence against women and poverty. Has a strong alignment with YWCA mission.

· Strong knowledge of community resources in Whatcom County; especially related to housing, or willingness to rapidly educate yourself.

· Must understand and embrace a respectful harm reduction, empowerment model of providing services.

· Spanish speaking preferred but not required.

Other:

· Valid WA State Driver’s license, valid insurance and 3 years drivers abstract.

· Washington State Patrol Background Check.

Physical Demands:

· Need to be able to ascend and descend multiple flights of stairs to assist residents in an emergent situation.

· Lift and carry up to 25 lbs.

· Regularly required to listen to and provide feedback to residents; sit for extended periods of time; be able to frequently stand, walk, climb or balance.

· Ability to move residents to and from housing areas. May include moving furniture in teamwork settings
