Zumbro Valley Health Center is seeking a flexible, team-oriented, and highly motivated individual to join the Care Coordination team. As a Care Coordinator you will be responsible for coordinating integrated medical, behavioral, and substance use care for clients.

What you will be doing:
- Coordinate multidisciplinary treatment plan meetings with client, family, and external providers.
- Coordinate communication between client, primary care, behavioral health and other service providers.
- Facilitate, connect, and monitor internal and external referrals.
- Conduct basic health screenings; provide education and referrals for preventative and other wellness service.
- Coordinate discharge/transition planning for clients admitted to the hospital or emergency room, assist with scheduling follow-up within state guidelines, complete required documentation.
- Document and monitor progress in achieving client's goals.
Qualifications
Qualifications
- Associate or Bachelor’s degree in Social Work, Psychology, Nursing or related field required
- 1 year (2,000 hours) of experience working in the mental health or behavioral science field.
- Must qualify as a Mental Health Practitioner
- Valid Driver's License and Proof of Auto Insurance

Starting wage $50,000 to $54,000 per year dependent on experience.
