We are seeking a Navigator Specialist to join our Intake team! You will provide and coordinate integrated medical, behavioral health, and substance use care for clients.
Responsibilities
- Provide individual sessions to clients assigned to caseload during their transition into programming.
- Conduct basic health screenings, provide education, and make referrals for additional services as needed.
- Assist with outreach and engagement of clients.
- Facilitate and monitor internal and external referrals.
- Meet with clients following hospitalization or ED visits.
- Complete all required documentation and paperwork within designated timeframe.
Qualifications
Requirements & Qualifications
- Bachelor's degree in Psychology, Social Work, or a Behavioral Health related field
- 1-2 years of experience working in the mental health field
- Must qualify as a Mental Health Practitioner in MN
- Valid Driver's license and Proof of Auto Insurance

Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.
