Zumbro Valley Health Center is seeking a Health Coordinator to join our Residential Treatment team! The Health Coordinator is responsible for providing direct care to clients residing within the residential treatment and crisis stabilization setting. The Health Coordinator monitors client medication administration, ensures accuracy of the daily MARS, provides client care coordination and acts as a liaison between other unit staff and the RN.

We are one of the few providers in the state, to provide such an integrative and comprehensive care to all children, adults, and families. In order to provide an integrative approach, we offer a wide variety of services including Outpatient Psychotherapy and Substance Use, Inpatient Residential Treatment, Withdrawal Management and Detoxification, Housing, Skills – Based Training and Targeted Case Management for adults and children, and more.
Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.

Responsibilities
- Keep track of stock supplies within the medication room on a monthly basis to ensure adequate supply.
- Assist with completing client rehabilitation notes.
- Communicate courteously and effectively with co-workers, supervisor, and other community resources.
- Monitor medication passing when present on site.
- Provide quality assurance and reconciliation over MARS.
- Complete medication error document for any errors discovered during shift.
- Responsible for reordering client medications weekly and ensuring on time arrival.
- Responsible for recording all medications that arrive on the unit during shift.
- Complete an initial health screen on all new client admissions.
- Assist treatment team in with care coordination when involving medications.
- Act as a liaison between treatment staff and the Registered Nurse.
- Assist the Registered Nurse with medication destruction.
Requirements and Qualifications
- Bachelor’s degree in Social Work, Psychology or a related human services field.
- 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
- Must qualify as a Mental Health Practitioner in the State of MN
- Licensed as a Social Worker in the State of MN preferred.
- Provisional License in the State of MN will be considered.
- Experience working with severe emotional disturbances in children is preferred.
- Valid driver's license and proof of auto insurance.
