Key Responsibilities
- Conduct home visits for patients enrolled in the Chronic Care Management Advanced Care program (ACP) and the Chronic Care Management program (CCM).
- Perform initial comprehensive evaluations and follow-up visits aimed at identifying and addressing risk factors for disease exacerbations and hospitalizations.
- Collaborate closely with other healthcare professionals to ensure a unified approach to patient care.
- Share after-hour telephone on-call responsibilities with fellow licensed providers.
- Document all clinical encounters, symptoms, and care goals in the electronic medical records (EMR) system promptly and thoroughly.
- Educate patients and their families regarding chronic medical conditions, preventive care practices, and medication adherence.
- Strong clinical assessment and analytical abilities.
- Excellent leadership and interpersonal communication skills.
- Proficient in electronic medical records and familiar with Microsoft Office applications.
- Ability to effectively collaborate within a diverse healthcare team.
- Highly motivated self-starter with exceptional organizational skills.
- Capacity to articulate complex medical information to patients and families.
- Graduate from an Advanced Nurse Practitioner program focusing on Adult, Geriatric, or Family Nurse Practitioner.
- Prior clinical experience in palliative care, hospice, oncology, ICU, or geriatrics is preferred.
- Experience in home-based care is advantageous.
- Valid state driver’s license is required.
- Demonstrated proficiency in diagnosing, treating, and documenting acute and chronic diseases in adult patients.
