Social Worker (WEST)

Job Description:
We are looking for a compassionate and detail-oriented Social Worker to support and safeguard the well-being of our clients. You will play a vital role in engaging families, conducting safety assessments, and ensuring compliance with care plans.

Key Responsibilities:

• Engage and build rapport with caregivers and children from diverse backgrounds.
• Provide timely support and interventions based on case and care plans.
• Conduct in-person and virtual safety checks to assess well-being.
• Maintain accurate case notes, documentation, and prepare social reports.
• Identify and report breaches in safety or care plans.
• Meet all scheduled visitations and follow up on intervention plans.
• Continuously review and improve casework interventions.

Requirements:

• Strong interpersonal skills to engage and support caregivers of all ages.
• Empathetic, emotionally resilient, and adaptable in handling challenging situations.
• Minimum Diploma in a relevant field.
• Prior experience in social work or social service is required.
• Open to travelling around Singapore.
