Job Scope

• Provide psychosocial care and case management under the guidance of the Medical Social Worker.
• Assess and process financial assistance applications for patients and their families.
• Conduct home visits and assess living conditions for follow-up by the Medical Social Worker.
• Prepare brief social assessments for continued case management.
• Support group work sessions, including logistics coordination and co-facilitation.
• Participate in community mapping and engagement efforts.
• Assist in planning and execution of community events.
• Contribute to departmental education, quality improvement projects, and professional ethics adherence.
  
  

Job Requirements

• Minimum GCE 'O'/'A' Levels or NITEC in Community and Social Services.
• At least 2 years of relevant experience in social services is an advantage.
• Strong interpersonal and communication skills to engage with patients and families.
• Ability to work independently and collaborate with multidisciplinary teams.
• Organized, proactive, and committed to improving patient outcomes.
