Social Work Associate

Responsibilities:

Referrals & Admissions

• Assessment of all referrals for admission / rejection according to criteria, and flag up any outstanding issues with Medical Social Worker / Senior Medical Social Worker for discussion
• Assists in consolidating and providing tracking of updated information for referral assessment as requested
• Works with and supports Medical Social Worker in preparation of admission documents
• Supports Medical Social Worker / Senior Medical Social Worker during resident’s admission, in following up with processing activation of financial payout schemes

Psychosocial Support and Groupwork

• Assists in simple case management of residents with non-complex family dynamics or social issues
• Works with Community and Partnerships, and Allied Health professionals to develop care plan initiatives for resident/family engagement
• Assists the Medical Social Worker in Groupwork Therapy Sessions

Case Conference and Discharge

• Prepares and consolidates information ahead of case conference
• Prepares supporting documents for referral to step down care
